All Assemblies in the Northern Illinois District are invited to participate in this
Exemplification

•  Candidates need to be present at 10:30 am. 
• Deadline for Banquet is 4/25/2014 Deadline for Candidates Form 4's for names to be listed in the Souvenir Program Book is 4/12/2014.
• SPECIAL REMINDER a candidate must be a 3rd Degree Member in good standing to be eligible for the 4th Degree Exemplification.
• Please be sure to send only completed packets (Form 4, Dinner Reservation Form, and check made payable to the Master at least 10 days prior to the Exemplification Date).

Candidates initiation fee $70.00 - includes Banquet Ticket, Social Baldric, and 4th Degree Pin.

INCOMPLETE PACKETS OR PACKETS NOT HAVING ALL THE INFORMATION WILL BE RETURNED TO THE FAITHFUL NAVIGATOR.

 Please note that Candidates should be reminded to have their current 3rd Degree Membership Card with them at the time of registration.
